Religious School Registration 2020 - 2021

STUDENT INFORMATION




Washington Hebrew Congregation offers highly subsidized Religious School tuition for members of our congregation. In addition, we open our school community to those who are not WHC members at non-subsidized rates for students in our Pre-K program through 3rd grade. To enroll a child in grades 4 through 7, families must be WHC members.






Please indicate the Religious School grade your child will be in next year and where they will attend Religious School.
The tuition listed alongside the grade will increase on May 18, 2020.

Please list up to two (2) social requests (first and last name), in order of preference. Please note that while we will do our best, these are not guaranteed due a number of variables including when registrations are submitted.

CONSENT & LIABILITY WAIVER

As lawful consideration for my child being permitted to participate in the activities of the Washington Hebrew Congregation Religious School  (the “School”), including, without limitation, any transportation provided to my minor child by an employee, officer, director, volunteer, agent or servant of the School (each, an “Authorized Person”), I agree that neither my minor child nor I, nor our respective heirs, assigns, personal representatives or estates, may or will make any claim against, sue or attach the property of the School or of any Authorized Person for damages by reason of death, personal injury, accident, illness or property damage which I or my minor child may sustain as a result of his or her participation in these activities. This release is intended to discharge in advance the School and all Authorized Persons from and against liability, including for negligent acts or omissions, except liability arising out of willful or wanton misconduct by such Person. I further attest that my child is physically fit and has no known medical conditions which would inhibit his or her participation in such at the School and all Authorized Persons.


Should it be necessary for my minor child to have medical treatment while participating in the School activities while under the supervision of an Authorized Person, I give permission to such Authorized Person(s) to use their judgment in obtaining medical services and I give permission to the physician selected by such Authorized Person to render medical treatment as such physician deems necessary or appropriate. I understand the School may not carry insurance covering such medical or hospital costs and agree that any such costs shall be my sole responsibility.


I understand that WHC is relying upon the statements made by me on behalf of my child in this Release.

I am at least 18 years of age and am a parent or legal guardian of the minor. I hereby consent and agree that my child and I will be bound by this Release. In addition, to the extent the recording, live streaming and subsequent use of the Appearance, as contemplated by the Release, may constitute the collection, use or disclosure by WHC of my child’s personal information, I hereby provide my permission to WHC. 

I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS.  I AM AWARE THAT THIS IS A RELEASE OF LIABILITY FOR MYSELF AND MY CHILD AND A CONTRACT BETWEEN MYSELF, MY CHILD, AND THE SCHOOL AND ITS EMPLOYEES, OFFICERS, DIRECTORS, VOLUNTEERS, AND AGENTS, AND I HAVE SIGNED IT OF MY OWN FREE WILL.

CONSENT FOR WEB STREAMING

I acknowledge that my child will be participating in Washington Hebrew Congregation Services and Programs (each, an “Event”) throughout 2020-2021. By signing this Release and Consent (this “Release”), I authorize Washington Hebrew Congregation and its affiliates, representatives, successors and assigns (collectively, “WHC”) to video and photograph my child and/or record my child’s voice, conversation, likeness, image, sounds, and appearance during an Event.


I agree that WHC will be the exclusive owner of all right, title and interest in and to such videography, photography and/or recording. WHC may use, and license others to use, my child’s voice, conversation, likeness, image, sounds, and appearance, (collectively, the “Appearance”), throughout the world, an unlimited number of times in perpetuity, in any medium, including the Internet, in connection with any WHC product or service and any promotional, advertising, marketing or publicity activities of WHC. In addition to all other rights, WHC will have the right to live stream an Event over the Internet and to archive and replay in the future, at any time, without limit, any recording of an Event, in whole or in part.

I waive any right of inspection or approval of my child’s Appearance or the uses to which it may be put by WHC.

I acknowledge that WHC has not made any representation or warranty of any kind with respect to my child’s participation in an Event, his/her Appearance or the subsequent use of any recording thereof.



PHOTO/VIDEO CONSENT & RELEASE

This next section explains how Washington Hebrew Congregation (“WHC”) and the Religious School may use photographs and video/audio recordings (“images”) taken while your child is participating in our programs. It allows you to state your preference(s) for how we may use and share any images of your child. 


This form covers images of your child captured while your child is at Religious School. It does not include images taken at WHC programs/events that are open to the public, such as holiday services (e.g., Shabbat Hanukkah) and Mitzvah Day. 


The opt-in and opt-out preferences you indicate below cover the 2020-2021 school year. However, with your consent below, images of your child may be used for promotional purposes or in WHC or Religious School social media after the 2020-2021 school year. 


INTERNAL USE

Visual and audio documentation of students’ learning experiences are a vital part of the engaging curriculum at Religious School. Documentation enables teachers to intentionally plan curriculum and sharing images of children with their parents facilitates teacher-parent communication about daily school activities as well as the child’s development over time. 

 

I understand that by enrolling my child in WHC's Religious School, I consent to allow Religious School faculty to photograph, record, and/or film my child while they are participating in Religious School events and programs, whether held at Temple, the Julia Bindeman Suburban Center, or at an location. during the school day for educational purposes, curriculum documentation, and parent communication. 

 

I also understand that images of my child may be displayed in Religious School classrooms and school hallways, and teachers may email them to their classroom families via their WHC email account. 


PROMOTIONAL USE

WHC and the Religious School promote programs and events to the Temple community as well as the general public. To do so, they may use images of students taken during Religious School (as described in the “Internal Use” section above) in print and digital materials, such as brochures, flyers, posters, banners, newsletters, email, advertisements, and the WHC website. 


WHC and the Religious School do not include children's names and/or personally identifiable information in promotional materials. 


Do you consent to allow WHC and the Religious School to use images, videos, and recordings of your child for Promotional Use as described above?



SOCIAL MEDIA
The Religious School has a public Facebook page. Additionally, WHC has a public Facebook page, Instagram profile, and a YouTube account where we regularly share information and images to strengthen and engage the community and promote our programs and events that are open to the public. Images are not “tagged,” and no names or personally identifiable information are included in any post.

Do you consent to allow WHC and the RJWECC to use images of your child for Social Media as described above?

RELEASE
I hereby release and discharge WHC, the Religious School, and their successors and assignees from all and any claims and demands ensuing from or in connection with their use of images of my child for the purposes described in this Consent and Release Form. 



SCHOOL DIRECTORY
We publish a Religious School directory each year. Please indicate whether or not you wish to list your child and include your contact information in the 2020-2021 directory.

SUMMARY OF 2020-2021 FEES

In order to keep Religious School fees as low as possible, WHC subsidizes the cost for our members. All families pay an annual Religious School security fee: $35 per WHC family and $180 per non-member family. Your total deposit appears below.


Please note a $1,700 b'nei mitzvah fee is not included below and will be billed by the Cantors' office. It can be paid in annual installments over three years leading up to your child's b’nei mitzvah or in-full. This fee covers the b’nei mitzvah tutoring sessions, materials, rehearsals, the Oneg Shabbat  (food following the Friday evening service) and bimah flowers.

UPPER SCHOOL MEAL DONATION
As part of our weekly Upper School/Confirmation programming for grades 8-12, we provide dinner from 6:30 - 7:00 pm to any student who is interested. In order to continue providing a variety of meals that students truly enjoy, please help us offset the costs of these dinners by making a voluntary $50 contribution. We greatly appreciate your support and know that the time set aside for meals is also a great social and community building experience for our teens.
