As lawful consideration for my child being permitted to participate in the activities of the Washington Hebrew Congregation Religious School  (the “School”), including, without limitation, any transportation provided to my minor child by an employee, officer, director, volunteer, agent or servant of the School (each, an “Authorized Person”), I agree that neither my minor child nor I, nor our respective heirs, assigns, personal representatives or estates, may or will make any claim against, sue or attach the property of the School or of any Authorized Person for damages by reason of death, personal injury, accident, illness or property damage which I or my minor child may sustain as a result of their participation in these activities. This release is intended to discharge in advance the School and all Authorized Persons from and against liability, including for negligent acts or omissions, except liability arising out of willful or wanton misconduct by such Person. I further attest that my child is physically fit and has no known medical conditions which would inhibit his or her participation in such at the School and all Authorized Persons.

Should it be necessary for my minor child to have medical treatment while participating in the School activities while under the supervision of an Authorized Person, I give permission to such Authorized Person(s) to use their judgment in obtaining medical services and I give permission to the physician selected by such Authorized Person to render medical treatment as such physician deems necessary or appropriate. I understand the School may not carry insurance covering such medical or hospital costs and agree that any such costs shall be my sole responsibility.

I understand that WHC is relying upon the statements made by me on behalf of my child in this Release.

I am at least 18 years of age and am a parent or legal guardian of the minor. I hereby consent and agree that my child and I will be bound by this Release. In addition, to the extent the recording, live streaming and subsequent use of the Appearance, as contemplated by the Release, may constitute the collection, use or disclosure by WHC of my child’s personal information, I hereby provide my permission to WHC. 

I acknowledge that my child will be participating in Washington Hebrew Congregation Services and Programs (each, an “Event”) throughout 2024-2025. By signing this Release and Consent (this “Release”), I authorize Washington Hebrew Congregation and its affiliates, representatives, successors and assigns (collectively, “WHC”) to video and photograph my child and/or record my child’s voice, conversation, likeness, image, sounds, and appearance during an Event.

I agree that WHC will be the exclusive owner of all right, title and interest in and to such videography, photography and/or recording. WHC may use, and license others to use, my child’s voice, conversation, likeness, image, sounds, and appearance, (collectively, the “Appearance”), throughout the world, an unlimited number of times in perpetuity, in any medium, including the Internet, in connection with any WHC product or service and any promotional, advertising, marketing or publicity activities of WHC. In addition to all other rights, WHC will have the right to live stream an Event over the Internet and to archive and replay in the future, at any time, without limit, any recording of an Event, in whole or in part.

I waive any right of inspection or approval of my child’s Appearance or the uses to which it may be put by WHC.

I acknowledge that WHC has not made any representation or warranty of any kind with respect to my child’s participation in an Event, their Appearance or the subsequent use of any recording thereof.

This next section explains how Washington Hebrew Congregation (“WHC”) and the Religious School may use photographs and video/audio recordings (“images”) taken while your child is participating in our programs. It allows you to state your preference(s) for how we may use and share any images of your child. 

This form covers images of your child captured while your child is at Religious School. It does not include images taken at WHC programs/events that are open to the public, such as holiday services (e.g., Shabbat Hanukkah) and Mitzvah Day. 

The opt-in and opt-out preferences you indicate below cover the 2024-2025 school year. However, with your consent below, images of your child may be used for promotional purposes or in WHC or Religious School social media after the 2024-2025 school year. 

WHC subsidizes the cost of Religious School for Temple members. Religious School tuition includes b'nei mitzvah training and preparation fees for all grades with the exception of Independent Studies (it does not include floral and oneg fees). In addition to tuition, all families pay an annual Religious School security fee: $100 per member family, $150 per non-member family.
